Notice of Privacy Practices (HIPAA)

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

1. Who we are

Lotus Wellness is a Targeted Service Coordination provider. As a covered entity under the Health Insurance Portability and Accountability Act ("HIPAA"), we are required by law to maintain the privacy of your Protected Health Information ("PHI") and to provide you with this notice of our legal duties and privacy practices.

2. How we may use & disclose your PHI

For treatment: coordinating with providers, schools, and supports involved in your care.
For payment: to obtain payment from Medicaid or other payers for the services we deliver.
For health care operations: quality review, staff training, audits, and case coordination.
As required by law: mandated reporting (e.g., suspected abuse or neglect), court orders, public health.
With your written authorization: for any other purpose, including marketing or sharing with third parties not listed above. You may revoke an authorization in writing at any time.

3. Your rights regarding your PHI

Inspect and obtain a copy of your record.
Request an amendment if you believe it is inaccurate.
Receive an accounting of certain disclosures we have made.
Request restrictions on how we use or disclose your PHI (we will consider, but are not always required to agree).
Request confidential communications (e.g., a specific phone or mailing address).
Receive a paper copy of this notice, even if you have agreed to receive it electronically.

4. Our responsibilities

We are required by law to maintain the privacy of your PHI.
We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
We must follow the practices described in this notice, and reserve the right to change them. Material changes will be posted here with a new effective date.

5. Filing a complaint

If you believe your privacy rights have been violated, you may file a written complaint with us at callie@lotustsc.com, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
